One-Time Release of Information Form:

• For a student who is 18 or older and who is enrolled in any Independent Study course (high school or college course) who wants to allow specific information about their academic record to be released to a specific person or agency.

• For a student who is under 18 years old who is enrolled in a college course and wants to allow specific information about their academic record to be released to a specific person or agency.
